DOSAGE AND ADMINISTRATION
Dosage
Adults
The usual adult dosage is 1–4 g per day, administered in pided doses. Most infections respond to 500 mg every 8 hours For skin and soft tissue infections, streptococcal pharyngitis, and mild, uncomplicated urinary tract infections, the usual dosage is: 250 mg every 6 hours, or 500 mg every 12 hours.
In cases of severe infections or infections caused by less susceptible organisms, higher doses may be required. If a total daily dose exceeding 4 g is indicated, consideration should be given to parenteral administration at an appropriate dosage.
Elderly patients and patients with renal impairment: Dosage is the same as for adults.
In patients with significant renal impairment, dose reduction is required.
Pediatric population: The recommended usual daily dose in children is 25–50 mg/kg/day, administered in pided doses. For skin and soft tissue infections, streptococcal pharyngitis, and mild, uncomplicated urinary tract infections, the total daily dose may be pided and administered every 12 hours.
Recommended dosing for most infections:
Children under 5 years: 125 mg every 8 hours . Children 5 years and older: 250 mg every 8 hours
In severe infections, the dosage may be doubled.
In the treatment of otitis media, clinical studies have shown that a dosage of 75–100 mg/kg/day, administered in four pided doses, is required.
